No luck yet. Never could get it to even show up as mutator in any mode.
Haven't had the chance to contact any other server admins to try and peek at their server ini files.
Will post an ini file for a vanilla chaos server when I get it up and running.

Thanks for the replys. I have tried all of those things with no luck. I guess the next step is to contact the admins of the few remaining servers and try and get a copy of their ini files and/or send a help ticket into game servers and see what they say.
